Just read that the first passenger elevator safety brake was invented because a guy saw a rope snap

Found it in a museum pamphlet in Chicago last week. How many lives do you figure that one observation has saved over the years?

Honestly, @graceblack is right, we probably miss stuff like that all the time. Tbh, it reminds me of those little plastic tabs on car windows so kids can't strangle themselves. Someone had to see that weird, specific danger first. Makes you look at every normal thing and wonder what deadly flaw is just sitting there waiting for someone to notice.

Forget the dangers we see, what about the ones we cause?

That's a wild story. It's crazy how many huge safety things come from someone just noticing a small, broken detail. Makes you wonder what we're all walking past today that needs fixing.
