Had a controller lock up on me at a hospital in Cleveland last Tuesday
The car was full of people and I had to emergency stop it on the 4th floor, then spent 45 minutes with a multimeter tracing a bad solder joint on the main board that nobody taught me about in trade school - is this something you guys run into more often on older Otis units or am I just unlucky?

Yeah Otis units from the late 90s before they switched to the MCP boards are notorious for cold solder joints on the main processor board. I have seen it on at least 15 different jobs in the last 5 years. It usually shows up as random controller lockups or intermittent callbacks that seem to fix themselves. The problem is the solder cracks from heat cycling and vibration over time. You can fix it by reflowing all the big connections with a decent iron and some fresh solder. Don't waste time replacing the whole board unless you have to, most places will charge you 2 grand for a refurb.

Hows the heat on those old Otis boards by the main processor? Ive heard some shops say adding a little fan helps prevent the cracking in the first place.
