Swivel bearing went bad on a Friday afternoon job

Last month I was lifting a 12 ton HVAC unit onto a roof in downtown Phoenix. About halfway through the pick I felt a weird vibration through the cab. Finished the lift but the swivel was grinding bad on the way down. Turned out the bearing race had cracked and lost all its grease. Had to shut down for two days to get a replacement from the supplier. Anyone else had a swivel bearing fail without any warning signs?

Man, I once stripped a bearing so bad it sounded like a bag of marbles in a blender, oops.
