Debate: Is 2 Inches of Creosote the Real Danger or Is It the Flue Temp?

Had a chat with an old-timer at the supply house in Columbus last week. He swore the only number that matters is buildup thickness, not the temperature reading. I've always gone by the 1/4 inch rule for glazed creosote, but he pulled up a job where 2 inches of fluffy stuff caused a liner failure in a cabin near Lancaster. Meanwhile, my partner says if flue temp stays below 250 for the first hour, you're asking for trouble no matter what. Which side do you lean on, and have you seen a case where thickness or temp surprised you? I'm trying to settle this before I redo my inspection checklist.
