Almost lost a finger to a loose dado stack last Tuesday

I was in my shop near Richmond running a stack of plywood through the table saw with a dado blade set, trying to knock out some shelf dados on a big bookcase order. Mid-cut something felt off, the blade started chattering real bad, and I shut it down quick. Took a look and found the outer blade had worked loose because I'd been lazy and not torqued the nut down enough when I swapped the setup. Could have been a lot worse, the blade nearly walked right off the arbor. Now I double-check with a wrench every time, even if I'm in a hurry. Anyone else had a close call with a blade coming loose on a job?

Ngl that gave me chills just reading it. Honestly I've had a blade shift on me once during a ripping cut on some cherry and it scared me bad enough that I still think about it every time I start the saw. A dado stack coming loose though that's a whole other level of terrifying since you've got so much more mass spinning. Man I'm glad you caught it before it actually flew off.
