Flange face blowout on a 12-inch line in Jersey City

We were hydro testing a feedwater line last month and I had the torque wrench set for what I thought was right. At about 900 psi the gasket just let go, water shot across the whole bay and almost took out a guy on the scaffold. Turns out the flange faces had pitting I missed in the dark, and I didn't run a dial indicator on the gap like I usually do. Spent 4 hours grinding and re-welding the flange before we could retest. Any of you run into hidden face damage that only shows up under pressure, and how do you catch it before the test?
